‘My Eyes Were So Puffy I Was Unable to Even Open Them’

Onemanda Preisinger is nervous about her daughter’s impending 13th birthday party. Not for the typical reasons associated with a home packed of loud adolescent children, but since it’s the initial occasion she will debut her recently altered face to acquaintances and relatives. “Clearly I’m going to tell all guests as they arrive, ‘For your information, this is not the way I appear,’” states the thirty-year-old property agent from Southern Florida.

Her appearance is, admittedly, a little startling – her face puffed up and unnaturally tightened, as though secured by industrial-grade tape. Her altered – and she’s eager to emphasize, short-term – look is the outcome of six cosmetic procedures, including an minimally invasive facelift, performed by a doctor in Istanbul, Turkey, last month. “My poor husband became emotional when he viewed me for the initial time because I couldn’t even unseal my eyes. That indicates puffy I was,” she explains to me via online call from her home. “I had to tell him: ‘Babe, I’m fine, I’m not in pain. I just appear as if someone jumped me.’”

Growing Trend of Early Facelifts

According to cosmetic specialists, Preisinger is among a rising count of people electing to undergo a facelift in their twenties and thirties – significantly before a ten years before most doctors’ usual candidate age of 40 to 60. (Although most surgeons are eager to highlight the professional guideline of: “We address heredity, not years.”) “I have individuals in their late twenties requesting facelifts,” says based in London aesthetic plastic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the head and neck. “It’s a major surgery, and I’m a bit concerned when I see people choosing it as a lifestyle choice.”

Explaining the Facelift Procedure

A facelift, alternatively called a rhytidectomy, elevates the ligaments in the face that begin to droop as we age. “Our faces are structured a bit like layers of an onion,” says Kent-based face specialist Marc Pacifico. “Dermis and adipose on the surface, then a stratum of supportive tissue called the SMAS. Think of the facial framework as the soft tissue skeleton of the face. And that’s what ages us; that is what becomes lax and pulls the dermis downward with it.”

Once the preserve of moneyed older people, overuse of injectables – when excessive use of injectable gels stretches the face and causes laxity in the dermis – combined with the common adoption of slimming medications, low-cost surgical travel, and the development of new, celebrity-endorsed operative methods are driving a different type of customer towards this invasive surgery. Statistics show an eight percent rise in facial lifts over the last year in the UK; while a survey revealed that the percentage of younger facelift patients is growing, with one-third of procedures now conducted for patients aged thirty-five to fifty-five.

“Patients are now requesting to appear as the optimal form of themselves and obviously the techniques are evolving,” says Orfaniotis. At present, the operation du jour is the comprehensive facial lift, a technique evangelised by figures including Kardashian matriarch Kris Jenner, to style creator Marc Jacobs.

Deep Plane Technique

Where a conventional facelift involves lifting the SMAS, the deep plane facelift loosens the facial ligaments underneath, enabling surgeons to reshape the face by repositioning the underlying layers, and preventing the taut, pulled look sometimes wrought by more traditional techniques. “We’re not putting tension on the dermis because we’re targeting deeper, to the deeper tissue,” explains prestigious London plastic surgeon Rajiv Grover, who specialises in the advanced method. “It is the deeper tissue that is repositioned. The skin comes with it as a passenger.” Elevating the entire soft tissue layer as a single unit results in a more natural-looking and longer-lasting result. It also means that the surgery is no longer being used for the express purpose of youth preservation – or revitalization, as the cosmetic field terms it – but for “enhancement” and facial restructuring, too. “We receive numerous requests from patients aged 28 to 35 for this reason,” notes Grover.

Individual Experience

Preisinger had not intended on getting a facial lift – at least not at the thirty years old. But years of using hyaluronic acid dermal fillers in an effort at “facial balancing” meant that by the time she reached her late 20s, she was “botched”. “I didn’t do this because I desired to appear youthful,” she says. “I underwent it because the injectable harmed my face. I wish I had never using it. If I had avoided the injectable, I don’t think I would be in this situation right now.”

If injectable substances completely disappear has historically been a point of contention in the cosmetic field. “Studies have shown that they seldom completely dissolve,” says Grover, “but they move and can obstruct drainage pathways. One of the contributors to what we term ‘puffy appearance’ is also the reality that to some extent, the face gets somewhat fluid-filled because its ability to drain lymphatic fluid has been diminished.” Although studies into the topic is early-stage, cautionary statements on injectables’ potential impact on the body’s drainage have been released, and additional studies initiated. An esteemed face specialist, speaking on the condition of anonymity, mentioned he would avoid using injectables in a client because of the dangers they pose. “Many doctors avoid talk about this, because the manufacturers who produce injectables can be pretty aggressive.” He’s heard stories, he adds, of cosmetic doctors being landed with court orders after expressing worries.

Choice and Surgery

For Preisinger, once she started using injectables, she felt as if she had to keep replenishing it – particularly when it started to migrate to other parts of her face. She was only twenty-eight when a nearby cosmetic doctor in Florida suggested that a face and neck lift could be what she needed to finally step off the cycle of treatments. After 24 months, she ended up choosing between a list of recommended hotels in Istanbul, texted to her by the patient coordinator at her doctor’s clinic.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – better known as blepharoplasty – but her surgeon advised her that a facelift was the right solution for the looseness she was experiencing in her face. She booked a brow lift, a mid-facelift and a jawline surgery. The day before her operation, 24 hours post she’d arrived solo in the country, she opted to include a mouth enhancement. “Then he stated, ‘We’re going to elevate the under-eye bags.’ Then he said, ‘I think if we remove some cheek fat, it will sculpt your face.’ So I ended up including additional procedures,” she recalls. She paid $22,000 (sixteen thousand five hundred pounds) for the six-hour surgery, plus a three-day|
